I am so excited to work with you! It is my honor and passion to help people claim the healthy relationship with food and body that they deserve. Please read the following important information fully and complete the two action steps before booking an appointment:

  • All appointments are conducted via telehealth.

  • A credit card is required to hold an appointment and is stored securely through my EMR. 

  • Please provide at least 24 hours of notice to cancel or reschedule an appointment. In the case of a no-show or late cancellation, your credit card will be charged for the appointment at my self-pay rate ($175/55 minutes), as this time is saved for you and I am unable to bill insurance for this time.

  • I see patients on a weekly basis until they get better. My patients are usually with me for 3-9 months, depending on severity.


Two action steps to take before booking:

  1. Please verify your Medical Nutrition Therapy (MNT) coverage using this script. You are not required to do this step, but know that if your insurance rejects the claim I submit for any reason, the credit card on file will be charged for the cost of the appointment at my self-pay rate ($175/55 minutes). I am in-network with Aetna, Blue Shield of California, HealthNet, Medicare, and United Healthcare.

    • If I do not contract with your insurance carrier, I can provide you with a document called a superbill. You can submit this to your insurance and you may be able to get reimbursed if you have out-of-network benefits. In this case, payment is due at the time of service.​

    • For self-pay patients, payment is due at the time of service. For your convenience, I accept all credit cards, as well as HSA and FSA cards.

  2. Please have your or your child's PCP/NP/PA/DO/PsyD fax this referral form to me (or one of their own referral forms, if they prefer). If there is no official diagnosis, please check Z71.3 on the referral form before you send it to your healthcare provider. You should not need to book an appointment in order to get a referral - just call/email them and ask for it. A referral is required to book an appointment whether you are using your insurance or are self-pay; California law requires that Registered Dietitians obtain a referral. I manually accept and reject all appointment requests and if I have not received a referral, I will unfortunately have to reject the appointment request until I receive one.
